Enrolment & Fees

We ARE accepting new patients – our books are open.

We welcome new patients and families moving to the area to enrol with our practice

If you choose to enrol with Te Puke Medical Centre your fees will be less because we will receive a partial government subsidy for you.
You can only be enrolled at one medical practice, however you are free to attend any practice as a casual patient where you will pay the full unsubsidised fee.

If you wish to enrol with Te Puke Medical Centre please download the complete set of “Enrolment Forms” – (Set 1), fill them in and give them to our receptionist.   Please note,  you are required to bring in the appropriate identification for eligibility and New Zealand citizenship at the time of enrolling.
You will need a separate form for each family member.
